Partner Information Sharing and Request Form

Please note: this form is for use by agencies and professionals to share information with the Police.

Before completing the Partner information sharing form please read the below guidance.

Prior to completion of the form, practitioners should seek advice and support from their agency's safeguarding lead or service manager.

Guidance

The Information Sharing Report offers a multi-agency system of sharing information with Dorset Police to aid keeping people safe. It should be used to provide details of any concerns about people who pose a risk to, or target, groom or sexually abuse young people/vulnerable adults.

The document can be utilised to provide details of any concerns about individuals or groups who pose a risk to young and vulnerable persons within the community through exploitation, targeting and abusing sexually or through other criminal means such as county line drug lines drug supply

The form focuses specifically on the behaviours or actions of alleged, suspected or known perpetrators, and can act to corroborate information that is already suspected or support positive disruptive action.

When submitting the form, in line with the above guidance, please ensure that it is submitted in a timely manner and is accurate. Timely, relevant intelligence is more likely to support positive action than historic concerns.

As much detail as possible should be included on the form, including names, address, DOBs (where known). Checks to provide this information should be completed prior to the form being submitted.

This information can include low level, soft ‘whisperings’ and gut feelings, something that does not sit well with your or your co-workers but has no-where else to go. It can include information from third parties or anonymous sources.

Any information suspected to be false or malicious should be clearly highlighted as such.

If the information concerns young people/vulnerable adults you must also share the information with other practitioners you know that are involved with the person. If the person is known to Social Services, you must send a copy to the person’s Social Worker or Children’s Practitioner.

Please specify what is required, when and for what purpose  

Once completed, the form will be sent to Dorset Police's Intelligence Bureau for reviewing, assessing and inputting onto the system. This will be disseminated throughout the Force as necessary.

You must also share the information with other practitioners involved with the person. If the person is known to Social Services, you must send a copy to the person’s Social Worker or Children’s Practitioner.

Where the information provided may identify the witness/informant, Dorset Police will not take any action which would place that person at risk of harm.

If the information is about significant harm to the person then normal Social Care referral systems must be used to report these concerns.

NEVER assume someone else has passed on the information you have. Duplicate information is better than none.

What we do with the information we collect from the form

Question: What are the reason for collecting this information?

Answer: To safeguard people that are potentially at risk of exploitation and/or abuse.

Question: Where is the information stored once it has been emailed?

Answer: The information will be stored on Dorset Polices secure record management system.

Question: Who has access to this information?

Answer: Vetted members of staff

Question: How long is the information held for?

Answer: Will be held as long as it is relevant and will be routinely reviewed to see if it is still relevant.

Question: If a person wants to have the emailed information deleted, or removed from any Force database, or devices it has been saved to which email address do they send this request to and how long will the process take.

Answer: The request would be made the Data Protection Team who would respond to the individual as soon as possible and within the statutory period of one month unless an extension is warranted.
